OBJECTIVES: To compare association of microalbuminuria with outcomes in patients with different comorbidities. BACKGROUND: The risk of adverse outcomes associated with proteinuria has been found to be linearly decreasing with even low-normal levels of microalbuminuria. It is unclear if comorbid conditions change these associations. METHODS: We examined the association of urine microalbumin-creatinine ratio (UACR) with mortality and the slopes of estimated glomerular filtration rate (eGFR) in a nationally representative cohort of 298,875 US veterans. Associations of UACR with all-cause mortality overall and in subgroups of patients with and without diabetes, hypertension, cardiovascular disease, congestive heart failure and advanced CKD were examined in Cox models, and with the slopes of eGFR in linear and logistic regression models. RESULTS: Very low levels of UACR were linearly associated with decreased mortality and less progression of CKD overall: adjusted mortality hazard ratio and estimated glomerular filtration rate slope (95%CI) associated with UACR =200, compared to <5 mcg/mg were 1.53 (1.38-1.69, p<0.001) and -1.59 (-1.83, -1.35, p<0.001). Similar linearity was present in all examined subgroups, except in patients with CKD in whom a U-shaped association was present and in whom a UACR of 10-19 was associated with the best outcomes. CONCLUSIONS: The association of UACR with mortality and with progressive CKD is modified in patients with CKD, who experience higher mortality and worse progression of CKD with the lowest levels of UACR. Proteinuria-lowering interventions in patients with advanced CKD should be implemented cautiously, considering the potential for adverse outcomes

Prognostic relevance of a T-type calcium channels gene signature in solid tumours: A correlation ready for clinical validation

BackgroundT-type calcium channels (TTCCs) mediate calcium influx across the cell membrane. TTCCs regulate numerous physiological processes including cardiac pacemaking and neuronal activity. In addition, they have been implicated in the proliferation, migration and differentiation of tumour tissues. Although the signalling events downstream of TTCC-mediated calcium influx are not fully elucidated, it is clear that variations in the expression of TTCCs promote tumour formation and hinder response to treatment.MethodsWe examined the expression of TTCC genes (all three subtypes; CACNA-1G, CACNA-1H and CACNA-1I) and their prognostic value in three major solid tumours (i.e. gastric, lung and ovarian cancers) via a publicly accessible database.ResultsIn gastric cancer, expression of all the CACNA genes was associated with overall survival (OS) among stage I-IV patients (all pConclusionsAlterations in CACNA gene expression are linked to tumour prognosis. Gastric cancer represents the most promising setting for further evaluation

Ensemble Learning for Low-Level Hardware-Supported Malware Detection

Abstract. Recent work demonstrated hardware-based online malware detection using only low-level features. This detector is envisioned as a first line of defense that prioritizes the application of more expensive and more accurate software detectors. Critical to such a framework is the detection performance of the hardware detector. In this paper, we explore the use of both specialized detectors and ensemble learning tech-niques to improve performance of the hardware detector. The proposed detectors reduce the false positive rate by more than half compared to a single detector, while increasing the detection rate. We also contribute approximate metrics to quantify the detection overhead, and show that the proposed detectors achieve more than 11x reduction in overhead compared to a software only detector (1.87x compared to prior work), while improving detection time. Finally, we characterize the hardware complexity by extending an open core and synthesizing it on an FPGA platform, showing that the overhead is minimal.

Observation of a ppb mass threshoud enhancement in \psi^\prime\to\pi^+\pi^-J/\psi(J/\psi\to\gamma p\bar{p}) decay

The decay channel ψπ+πJ/ψ(J/ψγppˉ)\psi^\prime\to\pi^+\pi^-J/\psi(J/\psi\to\gamma p\bar{p}) is studied using a sample of 1.06×1081.06\times 10^8 ψ\psi^\prime events collected by the BESIII experiment at BEPCII. A strong enhancement at threshold is observed in the ppˉp\bar{p} invariant mass spectrum. The enhancement can be fit with an SS-wave Breit-Wigner resonance function with a resulting peak mass of M=186113+6(stat)26+7(syst)MeV/c2M=1861^{+6}_{-13} {\rm (stat)}^{+7}_{-26} {\rm (syst)} {\rm MeV/}c^2 and a narrow width that is Γ<38MeV/c2\Gamma<38 {\rm MeV/}c^2 at the 90% confidence level. These results are consistent with published BESII results. These mass and width values do not match with those of any known meson resonance.Comment: 5 pages, 3 figures, submitted to Chinese Physics
